Your privacy matters to us
FIREFLY is committed to protecting the privacy rights of our clients and their personal information. The Agency bases its privacy practices on the requirements of the federal Personal Information Protection and Electronic Documents (PIPEDA) and the provincial Personal Health Information Protection Act (PHIPA).
Collection of Personal Health Information
We collect personal health information directly from you, from someone authorized to act on your behalf and indirect from other sources, such as other care providers, schools and physicians, where we have obtained consent to do so or if the law permits. Examples of personal health information that we collect include your health history and records of care or treatment you may have received elsewhere. We will not collect personal health information if other information will serve the purpose, and we will not collect more personal information than what is necessary to meet the purpose.
Use and Disclosure of Personal Health Information
The Agency uses and may disclose your personal health information in order to:
Provide assessment, treatment, counseling, and other services.
Plan, administer, and manage our internal operations.
Conduct risk management and quality improvement activities.
Facilitate review by accrediting and governing bodies.
Conduct research and compile statistics.
Comply with legal and regulatory requirements.
Fulfill other purposes permitted or required by law.
We may share your personal health information with other health care providers for the purposes of providing health care services, and unless you tell us otherwise, we assume that we have your consent to do so.
Access & Correction of Records
You have the right to request access to and correction of your personal health information and you can do so by contacting your FIREFLY service provider or our Privacy Officer, identified below.
In the unfortunate event that your personal health information is stolen, lost, or accessed by unauthorized persons, we will inform you at the finest reasonable opportunity.
